Relationship with the Nominee (for example, if you are nominating your Father, select “Father” from the drop-down menu)
Nominee date of birth (DOB)
Email address of the nominee
Identify Proof Type (PAN Card, Aadhaar, Voter ID or Passport)
Identify Proof Number (so if you have selected PAN Card in Identity Proof Type, enter PAN Card number)
